Information this website may collect

The public registration form collects family contact information, each child’s legal name, date of birth and sex, preferred language, appointment selections, optional previous-practice information, and insurance details or insurance-card images. When a contact is a legal guardian, the form may also require a court order or adoption document.

The expecting-parent consultation form collects the expecting parent’s name and phone number, plus optional email, preferred contact time, expected due date, hospital, and preferred language. These registration and consultation forms do not request diagnoses, medications, allergies, medical history, or clinical concerns, and they do not send confirmation emails.

Separate ADHD assessment forms collect child identifying information, parent or caregiver details, and responses about behavior, school performance, medication use, and, where applicable, side effects. Teacher forms also collect school and teacher details and a verified school or work email address. Teacher email verification uses a generic code message containing no child information. The website does not email child information or assessment responses.

The website may also record technical information needed to protect and document a submission, including the submission date and time, internet protocol address, and the version of each notice displayed.

How the information is used

Lincoln Pediatrics uses registration information to review a new-patient request, arrange an appointment, prepare an internal registration packet, and communicate about administrative matters. ADHD assessment responses support the clinic’s assessment and follow-up of the child. Information is also used to operate, secure, audit, and troubleshoot the website form services.

Secure electronic submission and storage

Information submitted through secure website forms is encrypted in transit. Sensitive form content and uploaded documents are encrypted before storage in the website’s private storage systems. Access is limited to authorized clinic staff and service identities needed to operate the website and clinic workflow.

Information submitted through website forms is not sold. Lincoln Pediatrics may disclose information to service providers that support the clinic’s registration, assessment, storage, security, printing, and practice-management workflows; as required by law; or as authorized by the person submitting the information.

Text-message consent

Providing a mobile number does not by itself enroll a person in clinic-initiated text messages. Text consent on the registration form is optional and is not a condition of care. Mobile opt-in information and text-message consent will not be shared with third parties or affiliates for their own marketing or promotional purposes. Service providers may process messages only to support Lincoln Pediatrics’ messaging service.

Retention and deletion

Website submissions remain available while staff review and process them. Website form records, uploaded files, and temporary office copies of registration packets are scheduled for deletion 30 days after the related request is marked Completed or Not Accepted, subject to applicable legal, security, and operational requirements. Copies accepted into the clinic’s electronic medical record follow the clinic’s medical-record retention policies and are not deleted by this website cleanup.

Encrypted disaster-recovery backups are managed separately and may contain earlier copies after deletion from active systems. The 30-day website cleanup schedule does not apply to those backups.

You may choose not to use the online form and instead call Lincoln Pediatrics at (704) 735-1441. Existing patients should use the patient portal for portal services. Do not use this website, text messaging, or email for an emergency. Call 911 for emergencies.
